How is RCRSP Diagnosed?

A diagnosis of RCRSP (Rotator Cuff-Related Shoulder Pain) is not found on a computer screen; it is found through a detailed clinical assessment. We look at your history, specific pain triggers, and how your shoulder moves and functions, particularly during elevation and external rotation.

The Truth About Scans: It may surprise you to learn that ultrasound or MRI scans are often unnecessary for RCRSP. Many people without any pain show rotator cuff “tears” or age-related changes on imaging. We typically only recommend imaging if:

  • There was a significant trauma or sudden, severe weakness.
  • We suspect a large, full-thickness tear.
  • Your symptoms are not improving as expected with physiotherapy.

The Best Treatment: Exercise-Based Rehabilitation

Physiotherapy is the internationally recommended first-line treatment for RCRSP. Because shoulder pain is “multifactorial,” your recovery depends on more than just lifting weights. We create individualized plans that address:

  • Load Tolerance: Gradually increasing the shoulder’s ability to handle stress.
  • Movement Control: Improving coordination and shoulder stability.
  • The “Whole Person”: Addressing lifestyle factors like sleep, stress, and confidence in movement.

Why Exercise Works: Specific, progressive loading helps tendons adapt, modulates pain signals, and builds the neuromuscular control needed to keep the joint stable. Most patients see significant improvement within 6–12 weeks of a structured program.

Physio vs. Medical Management: Injections & Surgery

While medical interventions have their place, they are rarely the first step for RCRSP.

  • Injections: Corticosteroid injections can offer short-term relief, but they are not a “fix.” They do not improve strength and, if used repeatedly, can actually accelerate tendon degeneration.
  • Surgery: Generally reserved for large traumatic tears or cases where prolonged physiotherapy has not provided relief. For most non-traumatic cases, research shows that surgery does not provide better long-term outcomes than a well-structured physio program.
